WHO: Recognize up to four (4) staff members. All submissions will receive an award.

SUBMISSION DEADLINE: Monday, July 15, 2024

The Employee Recognition Awards will be presented at the Celebration Dinner on Tuesday, October 22, 2024, at the Saratoga Hilton Hotel.

Below is space for your Affiliate to recognize up to FOUR (4) staff members. All staff members that are submitted will receive an award.

Information Needed:

For EACH of your Affiliates’ staff honorees, we ask that the following are submitted along with the contents of this form:

 

  • Photos: Please submit at least one (1) and up to five (5) of EACH staff member to be recognized. Include photos of them with people they support/work with. Any candid photo(s) you would like to share that capture the essence of the award winner would also be great to include.
  • Staff Award Honoree Questionnaire: Completed by EACH staff honoree. The link to the employee questionnaire will be in your confirmation email upon completion of this form. Please be sure to send these to the staff members chosen for awards as soon as possible.
