06 Jun CP State’s CHOP Program Featured in Mother Cabrini 2024 Impact Report
CP State is proud to share that our Community Health Outreach Project (CHOP) was featured in the 2024 Mother Cabrini Health Foundation Impact Report. The story highlights the transformative impact of CHOP on the life of Kaliope Uz Hernandez, a 10-year-old from Verona, who is non-verbal and lives with a complex developmental disability.
Thanks to CHOP and funding from the Foundation, Kaliope received a state-of-the-art communication app on an iPad – technology not covered by Medicaid. With this device, Kaliope can now express her needs, emotions, and thoughts clearly and confidently. Her mother, Alexandria, shared that the device has strengthened Kaliope’s relationships, reduced frustration, and opened new doors in her education and home life.
CHOP, administered by CP State, provides care management and access to essential services for children with complex needs – especially those whose health and development are affected by social determinants. Through outreach, advocacy, and personalized support, CHOP ensures families like Kaliope’s don’t fall through the cracks.
